Information and Authorization Form Health Professions School Applicants

Please provide contact information where you can be reached between May 15th and September 1st if any questions arise about your letter or application. Correspondence will be through your Stevenson University email address.
Please provide a recent photo of yourself to the Health Professions Advisor. A photo is usually required for your application to a health professions school as well.
Please list the people from whom you have requested or will request letters of evaluation. For medical or dental school, it is suggested that two letters are from science faculty and two letters are from science faculty and two letters are from health care providers who have actually observed you in a health care setting. The other letter can be from an (nonscience) university faculty, staff, athletic coach, research member or an employer. Remember that submitting more letters is not always viewed favorably by the admissions office. For other health professional programs, the number of recommendation letters and the number of health care providers required to write the letters wil vary.
It is important that you select faculty who you have a developed and maintained a professional relationship with over the years. You may not want to ask a faculty member that you had as a freshman and have not continued to interact with this individual in subsequent years. Please do not wait until the last minute to ask individuals for letters of evaluation. A minimum of three weeks' notice to the letter writer is advised and please remember that this person most likely is writing letters for other students.
For students applying to medical or dental schools, all individual letters of recommendations are directly sent to the Health Professions Advisory Committee. Please do not ask those writing letters of recommendation on your behalf to submit them to AMCAS. Please note that the composite letter will be written only after the letters have arrived from all of the persons named above. The Health Professions Advisor will notify you shortly after the April 20, 2012 deadline if any of these individual letters are missing. At that time, you will have the option of (1) asking another person to write a letter for you, or (2) going ahead with the letters you have, provided that you have a minimum of three (two from science faculty).
For students applying to other health professions programs (veterinary, physical therapy, physician assistant, etc.), all letters of recommendations written by individuals must be submitted directly to the centralized agency or to the individual schools, depending upon the directions stated in your application. The HPAC will send the committee letter separately according to the directions stated in your application.
AUTHORIZATION
The following statement must be signed if you are to have the committee letter sent in support of your application:
For students applying to medical or dental schools: I hereby authorize the Health Professions Advisory Committee at Stevenson University to write a composite letter of evaluation to be sent on my behalf to the medical or dental schools to which I am applying for admission. I understand that copies of all of the individual letters from the above named persons will be forwarded, along with the composite letter, to the centralized agency who will then forward them to the individual schools. If a particular school does not participate in the centralized agency, then the letters will be sent directly to their admissions office. I have indicated which medical or dental schools I have applied to on the composite list (Forms H, I or J).
For students applying to veterinary, physical therapy, physician assistant or other health professions programs: I hereby authorize the Health Professions Advisory Committee at Stevenson University to write a committee letter of evaluation to be sent on my behalf to the health professions schools to which I am applying for admission. For the individuals listed above, they will directly send their own letter of recommendation to a centralized agency or to the individual schools, according to the directions in the application.
Copies of any evaluation/recommendation letters written by the Health Professions Advisory Committee or submitted to the HPAC may also be used in support of my candidacy for prizes or awards for which I may be nominated, or for fellowships, grants, or positions for which I may apply.
