This form is only for the use of RNBS students. All other students should use the request to take a course off campus form.

RNBS students should complete this form and click submit. A copy will then be submitted to the Student Support Coordinator of the RNBS program. This form must be submitted prior to the start of the semester in which the student wishing to take courses off campus.
